Important: This search finds providers of medical services enrolled with Indiana Medicaid. However, these providers may not be accepting new patients or patients in every Medicaid program.

After using this search tool to find a provider, be sure to contact the provider directly to see if they are accepting new patients with Medicaid coverage.

For members enrolled in Healthy Indiana Plan (HIP), Hoosier Care Connect, Hoosier Healthwise or Indiana PathWays for Aging (PathWays), contact the assigned health plan to find medical providers within that plan’s network. If the member’s managed care benefits include dental coverage, this site may be used to find dental providers but the provider must be called to confirm that they accept the member’s coverage type.

Hint:   Qualified providers for Presumptive Eligibility (PE) are approved to determine preliminary eligibility for individuals, so the individuals can receive temporary healthcare coverage until the state determines official eligibility.
