CMS Announces 2012 Provider Enrollment Application Fee of $523

The Centers for Medicare & Medicaid Services (CMS) has calculated the provider enrollment application fee for 2012 and set the fee at $523. This fee supercedes the application fee of $505 published in BT201151 and posted October 25 in News and Announcements on indianamedicaid.com. For more information, see BT201154.
