Electronic Health Records Survey
The Indiana OMPP is currently participating in planning
activities to create and implement the Centers for Medicare &
Medicaid Services (CMS) EHR incentive payment program. The Medicaid
EHR incentive program will provide incentive payments to eligible
providers and hospitals for efforts to adopt, implement, upgrade,
or meaningfully use certified EHR technology.
To better estimate our eligible population of providers and
hospitals, Indiana Medicaid is asking that you complete the survey
linked below. The information you provide will not be reported
individually and will not be considered a commitment of any kind.
Your answers will be combined with those of other providers to
obtain an understanding of the overall healthcare technology needs
of Indiana. The OMPP will report aggregated data to CMS. Your
response will not automatically qualify you for incentive
funds.
