Recertify Provider Enrollment Licenses and Other Certifications

Recertification of expiring licenses and other certifications is necessary for a provider to continue to be eligible for enrollment with the IHCP. If a provider is required to recertify, a notification is sent to the provider 60 business days prior to the end date of a provider's eligibility to participate.

Note: The instructions below do not apply to OPR providers. For information on recertifying an OPR enrollment, go to the Participating as an OPR Provider page.

Prior to the provider's eligibility end date, recertification requires only the submission of the IHCP Provider Enrollment Recertification of Licenses and Certifications Form along with updates to the expiring license/credential documents.

If a provider fails to recertify before their eligibility end date, the provider must reenroll with the IHCP by submitting a new IHCP Provider Packet in its entirety. The following providers are required to recertify:

  • All out-of-state (OOS) individuals must recertify on or prior to their license expiration date. The provider's renewed license is submitted for eligibility extension.
  • Provider type 01 - Hospitals that are OOS and are eligible for the IHCP, recertify on or prior to their license expiration date. Providers with an open-ended license submit their Joint Commission on Accreditation of Healthcare Organizations (JCAHO) certification that shows their approval period.
  • Provider type 08 - Groups/Clinics must recertify the licenses of all rendering providers. The provider's renewed license must be submitted.
  • Provider type 26 - Transportation providers of all specialties recertify based on the requirements listed on the IHCP Provider Type and Specialty Matrix. Transportation providers are required to recertify based on their insurance end date or the motor carrier's certificate end date.
  • Provider type 27 -Dentist providers with Specialty 276 - Mobile Dental Van submit renewed licenses for recertification.
  • Provider type 28 - Laboratory providers with Specialty 281 - Mobile Lab or 283 - Mobile Independent Diagnostic Testing Facility (IDTF) submit renewed licenses.
  • Provider type 29 - Radiology providers with Specialty 291 - Mobile X-Ray Clinic submit renewed licenses and certificates.

Basic Steps to Recertify

  1. Use the IHCP Provider Type and Specialty Matrix to determine recertification document requirements. The licenses or certifications required to recertify are the same as those required to enroll.
  2. Complete the IHCP Provider Enrollment Recertification of Licenses and Certifications Form.
    • Detailed instructions are included in the form.
    • Be sure to get appropriate signatures and include all supporting documentation required.
  3. The Recertification form is an interactive PDF file, allowing you to type information into the fields from your computer, save the completed file to your computer, and print the file for mailing.
  4. Before mailing, make a copy of the form and other documentation for your records.
  5. Place the form and required documentation in an envelope.
  6. Mail the form and other required documentation to the following address:

Processing Your Recertification

Please allow at least 20 business days for mailing and processing before checking the status of your recertification. After the HP Provider Enrollment Unit processes your recertification form, they will notify you of the results.

  • If the form is incomplete or the required documentation is not present, you will be contacted by the HP Provider Enrollment Unit in an attempt to complete the transaction. If, after contact with you, the recertification cannot be completed, the entire submission will be returned to you, with a letter stating the problem. You will need to make corrections and resubmit the recertification paperwork.
  • If the form is complete, it will be processed and you will receive notification.
